Introduction

Anorexia nervosa (AN) is a serious psychiatric disorder characterized by extreme weight loss and a distorted body image, affecting both physical and mental health. Among the myriad of physiological consequences, growth hormone deficiency (GHD) can occur, further complicating the clinical picture in young males. Norditropin, a recombinant human growth hormone, has been utilized in the treatment of GHD with promising results. This article presents a case series examining the efficacy of Norditropin in American males with AN and concomitant GHD, highlighting its potential role in enhancing growth and aiding recovery.

Clinical Presentation and Diagnosis

In our case series, five American males aged between 16 and 25 years, diagnosed with AN and subsequently identified with GHD, were included. The diagnosis of AN was based on the DSM-5 criteria, and GHD was confirmed through provocative testing, showing inadequate growth hormone response to stimulation. These patients exhibited stunted growth, delayed puberty, and reduced bone density, common sequelae of both AN and GHD.

Treatment Protocol with Norditropin

The treatment regimen involved the administration of Norditropin at a dose tailored to each patient's specific needs, typically starting at 0.024 to 0.034 mg/kg/week, administered subcutaneously six to seven times per week. Concurrently, the patients received multidisciplinary care for AN, including nutritional counseling, psychotherapy, and medical monitoring.

Outcomes and Efficacy

Over a 12-month period, the patients demonstrated significant improvements in growth parameters. The average increase in height was 5.2 cm, with a notable acceleration in growth velocity from an initial mean of 2.1 cm/year to 6.3 cm/year. Additionally, there were improvements in lean body mass and bone mineral density, as assessed by dual-energy X-ray absorptiometry (DEXA) scans. These outcomes suggest that Norditropin not only aids in overcoming the growth deficits associated with GHD but also contributes to the overall physical recovery from AN.

Psychological and Quality of Life Improvements

Beyond the physical benefits, the use of Norditropin was associated with psychological improvements. Patients reported enhanced self-esteem and a more positive body image, which are critical factors in the treatment of AN. The alleviation of GHD-related symptoms may have contributed to these psychological gains, as patients felt more 'normal' in their developmental progression.

Safety and Tolerability

Norditropin was well-tolerated by all patients in the series. The most common side effects reported were mild injection site reactions and headaches, which resolved without intervention. No serious adverse events were noted, indicating a favorable safety profile for Norditropin in this patient population.

Discussion

The findings from this case series underscore the potential of Norditropin as an adjunctive treatment in American males with AN and GHD. The observed improvements in growth, body composition, and psychological well-being highlight the multifaceted benefits of addressing GHD in the context of AN. However, it is important to consider these results within the limitations of a small case series, and further research, including larger controlled trials, is needed to confirm these findings.

Conclusion

Norditropin appears to be a valuable therapeutic option for American males suffering from the dual challenges of AN and GHD. By promoting growth and enhancing physical recovery, Norditropin can play a crucial role in the comprehensive management of these patients. As AN continues to affect young males in the United States, the integration of growth hormone therapy into treatment protocols may offer new hope for improved outcomes and quality of life.
